If the site is older tha three years than AFFIDAVIT ( Performa no. 3 ) is required.

AFFIDAVIT ( Performa no. 4 ) from the applicant is required.

Note:->The MW-4/MW-7 tender form downloard from the site must accomplish with tender form fee of Rs.100/-in the shape of demand draft in favour of Corporation Engineer,Municipal Corporation,Jalandar payable at Jalandhar.Without the tender fee the tender form will not be entertained/accepted for any works tender.
